In case you forgot the login details, you can do the following:

1. In case you remember your feedback username and password, log in with these and send a message to the administrator, where you ask to reset your password. This will only work if you used this feedback account for creating a content moderator account.
2. Send an e-mail to the web site owner with a verification code, which was sent to you when you created the account
3. Create a new content moderator account. If this is successful, the old moderator account will be deleted (try to avoid this option)
4. If you are the owner of the website, you may change your account status in MySQL
